ADMISSION PROCEDURE
 

Eligibility
 
bullet Only female candidates can apply.
bullet A candidate should have passed 10+2 Science or equivalent examination with Physics, Chemistry and Biology either as a compulsory or optional subject with aggregate marks of 50%.
bullet Applicant must not be below 17 years and above2 8 years of age as
on 30th June 2005.
bullet Candidate should be medically fit for the course subject to Medical Examination Conducted by Hi-Tech Hospital.
   
  Application & Prospectus
 
bullet Application and Prospectus can be obtained from the office of the principal by payment of cash for Rs. 200/-(or) DD in favor of "Hi-Tech Medical College & Hospital "payable at Bhubaneswar for Rs.25Q/- to receive by post.
 
bullet Candidates are required to submit their application along with required documents by registered post / by speed post / by hand only to The Principal, Hi-Tech School of Nursing, Pandara, Bhubaneswar, on or before 30.08.2005.
 

Receipt should be collected incase of hand delivery. In complete applications, applications received after the due date, applications not accompanied by required documents shall be rejected.

  Documents to be submitted along with application
 
bullet Photocopy of High School Certificate issued by board of secondary Education or its equivalent board with date of birth recorded on it. No other documents supporting date of birth shall be accepted.
 
bullet Photocopy of pass certificate and mark list of +2 Science or Equivalent Examination from the concerned Council University.(+2 science).
 
bullet Photocopy of conduct certificate issued by principal of institutions last studied.
 
bullet Photocopy of cast certificate issued by competent authority to support special category if any. (In case of Schedule Cast / Schedule Tribe & OBC).
 
bullet Photocopy of college or school leaving certificate
 
bullet Two passport size photograph. (One pasted on the application form at the space provided with signature of the candidate on face of it).
 
bullet Photocopy of nativity / Resident certificate issued by competent authority.
